WEMT Course Requirements
To participate must have current CPR and State or National EMT certification.
Please complete and send the following:
- WEMT Forms: (click here for pdf)
1. Module Application Form
2. Signed and witnessed waiver
3. Medical Information Form
- Bank check/money order $400 made out to "San Juan Island EMS"
- Copy of current EMT/National Registry/Medical Certification
- Copy of current CPR
San Juan EMS must receive these items 2 weeks prior to course.
